Responsibilities

The Hospital Chaplain, in accordance with the plan of care approved by the attending physician when applicable, assists the Interdisciplinary Group (IDG) in assessing and meeting the spiritual needs of the patient/patient's family and/or caregivers. The essential functions of the Hospital Chaplain are:

Essential Functions:

• Provides spiritual support for all patients/families/caregivers as requested.
• Provides pastoral care that respects diversity and differences, including, but not limited to, culture, gender, sexual orientation and spiritual/religious practices.
• Assesses patient's/family's/caregivers' spiritual needs through an initial visit or telephone contact/assessment.
• Provides spiritual/pastoral care to patients/families/caregivers experiencing loss and grief.
• Facilitates and attends trauma training to ensure patient and family support.
• Provides effective spiritual/emotional support for hospital staff and volunteers as needed.
• Communicates with patient's community spiritual support person as indicated/requested. Acts as a liaison to the spiritual community as indicated.
• All contacts related to a particular patient are documented in the patient's medical record.
• Changes in the POC or patient's condition are communicated directly to the IDG via phone, secure text, EMR and/or during IDG meetings.

Schedule: Per diem, on call between 4:00pm and 8:00am, some weekends.
