Lung Care Navigator - Oncology Information
Full Time, 8am-4:30pm

Responsibilities

The Lung Care Navigator manages all lung imaging incidental findings and the workflow of the Low Dose CT (LDCT) Lung Screening Program. Performs basic nursing skills, patient navigation and data management largely in the outpatient setting with a focus on enhancing both the patient experience and the quality of medical care provided by the Lung Program. The Lung Care Navigator will focus on navigating patients with lung nodules, lung cancer or those at high risk for lung cancer, tobacco addiction and related problems. Works with referring physician offices to manage follow-up testing. Manages all aspects of the CT Lung database, including data entry and statistical reporting. Performs program quality assessment.

The Essential Functions:

  • Acts as a patient liaison and performs program participation, scheduling and outreach.
  • Oversees all efforts related to guiding patients and families through the screening, enrollment and appointment process.
  • Schedules appointment and diagnostic studies and tracks status of patients in database.
  • Reinforces patient education and directs patients and families to available resources and supportive services.
  • Develops initiatives to eliminate barriers to care.
  • Establishes processes and standards that ensure consistent and optimal patient navigation.
  • In accordance with NCCN criteria, enrolls patients into CT Lung Screening Program and responds to questions appropriately.
  • Communicates with referring physicians regarding patient appropriateness for program and coordinates information with referral services.
  • Acts as a liaison between Pulmonary and other physician practices and hospital departments. Understands, facilitates and communicates effectively with these entities as it relates to each patient.
  • Performs patient follow-up management for patients already screened, and uses database to ensure appropriate follow-up appointments are scheduled and kept as recommended
  • Ensures referring physicians are updated about their patient's progress in the program in cooperation with treating physician.
  • Responsible for Database Management: Manages all aspects of the CT Lung Screening database, including inputting patient screening data, patient letters, data entry input of all patient management activities and statistical reporting.
  • Provide updates to Cancer Committee on program activity, statistics and recommendations for program enhancements.
  • Schedules and coordinates CT Lung Screening Steering Committee meetings, to include setting agenda and documentation of meeting discussion.
  • Recommends and implements program guidelines. Develops required tools, forms, training materials, etc.
  • Works closely with marketing to promote program outreach.
  • Participates in departmental and/or interdepartmental quality improvement activities.
  • Reviews test results and coordinates additional work-up.
  • Assists with shared decision-making and education for patients undergoing lung cancer screening.
  • Responsible for Nodule Board agenda and presentation.
  • The Lung Care navigator will also serve as the IRB administrative assistant, which includes attendance at IRB meetings, detailed record-keeping and providing administrative support to IRB members regarding all aspects of IRB requirements for membership, as well as clinical trial submissions.
 

Qualifications

EDUCATION AND EXPERIENCE:
  • Vocational diploma/nursing diploma minimum requirement and over 1 to 3 years of nursing experience required.
  • Oncology experience preferred, BSN preferred.


LICENSES AND CERTIFICATIONS:

  • Registered Nurse in PA – licensed, upon hire, required.
  • Basic Life Support (BLS) upon hire required.
