Nurse Practitioner, Palliative Care
Part Time, Days, Benefits-Eligible - 32 hours biweekly, 8-hour day shifts

Responsibilities

The Nurse Practitioner, Palliative Care is directly responsible to the collaborating physician and is a registered nurse prepared through a formal organized educational program. The Nurse Practitioner, Palliative Care provides a full range of healthcare services, including planning, organizing, coordinating and directing optimal patient care and outcomes to ensure quality patient care to the adult and geriatric patient that meets regulatory requirements.

Essential Functions:
  • Performs interview health histories. Sources may include, but are not limited to, adult patients, geriatric patients and/or families, and records requisitioned from Grand View Hospital, other healthcare providers or other facilities.
  • Performs physical exams.
  • Interprets data from the history, physical exam and diagnostic tests to identify actual or potential patient problems and needs.
  • Develops and implements palliative plan of care for the adult or geriatric patient.
  • Monitors responses to palliative plan and adjusts plan accordingly.
  • Collaborates with patient's physicians re: patient palliative plan of care.
  • Provides teaching, consultation, guidance, counseling and emotional support to patients and their families.
  • Provides formal and informal education to other healthcare providers as necessary and/or requested.
  • The Nurse Practitioner may order the following to evaluate, diagnose and treat the adult and/or geriatric patient:
    • Medications (as allowed by regulation and licensure)
    • Diagnostic tests
    • Referrals and consultations
    • Measurements, monitoring, procedures and treatments (as allowed by regulation and licensure).
  • Discusses advance care planning with patient/families as appropriate.
  • Collaborates with other members of Palliative team as indicated for patient plan of care.

This is a PART-TIME role of 32 hours biweekly, consisting of 8-hour shifts Wednesday–Friday. Wednesday is a required meeting day, 8:00am–4:30pm. Mixed role of inpatient consultation visits and home visits. Coverage area is a 15-mile radius around Grand View Hospital.
 

Qualifications

EDUCATION AND EXPERIENCE:
  • Master's degree required.
  • Over 1 to 3 years of RN or NP home care experience preferred.

LICENSES AND CERTIFICATIONS:
  • Nurse Practitioner, licensed upon hire, required.
  • Registered Nurse in PA - licensed, upon hire, required.
  • BLS certification required.
  • DEA license required.
  • Auto and driver's license required.
